San Antonio Nursing HH, is looking for qualified home health Physical Therapist.
The Physical Therapist is responsible for ensuring that patient care is coordinated, managed, and delivered appropriately.
Physical Therapist will provide patient care according to a written physician's plan of care.
QUALIFICATIONS for Physical Therapist:
PT License required Home Health experience preferred Current CPR Certification Valid Driver's License Valid Car Insurance Familiar with Kinzer software RESPONSIBILITIES of Physical Therapist:
Educate and document discharge planning prior to discharge, including communication patient and family.
Patient is provided with discharge documentation Document patient visit information, ensuring information is sufficient that following staff can determine a clear plan for next contact Administer treatment involving application of physical agents such as heat, cold, light, water and electricity or traction and massage therapy Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability, injury or loss of body part.
Performs patient assessment test, measurements and evaluations, such as:
range of motion and manual muscle tests, gait and functional analyses, and body part measurements, records and evaluates findings to aid in establishing or revising specifics of treatment programs Recommended Skills Cpr Electrical Installation Work Functional Analysis Health Care Massages Measurement And Metrology Estimated Salary: $20 to $28 per hour based on qualifications.
